- ENROLLMENT -
All termination requests must be in writing.
- Effective Date of termination (Retro Terms only go back 60 days).
- Visit SkyDrive [profile] to confirm all other lines of coverage.
- If request comes thru for only 1 line of coverage-check with the employer to see if they should be terminated from the others as well.
- Document the termination.
- (Print screen the confirmation report and save in the Client’s enrollment folder).
- Send a copy to the client.
- Remind client that employee is eligible for COBRA (PA Mini or Federal).*
