Terminations



All termination requests must be in writing.

  • Effective Date of termination (Retro Terms only go back 60 days).
  • Visit SkyDrive [profile] to confirm all other lines of coverage.
  • If request comes thru for only 1 line of coverage-check with the employer to see if they should be terminated from the others as well.
  • Document the termination.
    • (Print screen the confirmation report and save in the Client’s enrollment folder).
  • Send a copy to the client.

  • Remind client that employee is eligible for COBRA (PA Mini or Federal).*
